Women in the perimenopause transition experience changes in estrogen, progesterone, testosterone, DHEA, thyroid function (often subclinically), insulin sensitivity, inflammatory markers, and key nutrients — simultaneously. Testing one or two of these in isolation, at a random point in the cycle, using broad standard reference ranges, is like trying to diagnose an engine problem by checking only the gas gauge. ## What We Test, When We Test It, and How We Read It

Our standard comprehensive panel includes: estradiol, progesterone, total and free testosterone, DHEA-S, full thyroid (TSH, free T3, free T4, thyroid antibodies — TPO and thyroglobulin), fasting insulin, fasting glucose, HbA1c, hsCRP (inflammatory marker), vitamin D, B12, ferritin, and magnesium. That's fifteen-plus markers covering the hormonal, thyroid, metabolic, inflammatory, and nutritional domains that perimenopause affects.

Timing matters. Progesterone is most informative when drawn during the luteal phase — days 19 to 22 of your cycle — when it should be peaking. A random progesterone draw often misses the decline that's driving symptoms. If you're still cycling, we schedule your blood draw accordingly. This single detail separates our testing from most standard panels.

Interpretation is equally critical. We use functional optimal ranges — narrower, clinically meaningful ranges that identify dysfunction before it becomes pathological. A TSH of 3.8 is technically 'normal' but may be functionally suboptimal for you. A fasting insulin of 10 is 'in range' but may indicate early insulin resistance. A ferritin of 25 is 'not anemic' but may be depleted enough to cause fatigue. Standard reference ranges are designed to flag disease, not identify dysfunction.
